Personal Care Services

Personal care services are intended for those who need assistance with personal hygiene, grooming, dressing, exercise and other day-to-day life and routine tasks. Whether you need us for a short while, perhaps while recovering from an accident or illness, or long term due to age, disability or other health problems – Corewood Care can help.

Personal Caregivers can assist with (but are not limited to) the following:

  • Bathing, shampooing, shaving, mouth and skin care, dressing and grooming
  • Mobility, ambulation and transfers
  • Light household duties important to health care at home
  • Incontinence assistance and toileting
  • Light housekeeping, shopping and meal preparation
  • Caring for client surroundings as needed to maintain a safe environment
  • Reminding the client about medications that must be taken
  • Follows the care instructions as prepared by the registered nurse assigned to the client

Care Management

Our Care Managers are trained nurses and social workers who will develop a comprehensive, personalized care plan that offer you and your family safe and cost-effective care solutions.

Care managers can help you navigate complicated medical systems, help you maximize health insurance savings and benefits, supervise your care and help you manage medications, accompany you to medical appointments, and facilitate regular family care conferences.

We can even provide relocation services, home safety assessments, wellness and safety checks, and referrals to specialists and local services. Regardless of your living situation, Corewood Care Managers will provide the professional expertise needed to enhance your physical, social, and emotional wellbeing.

Companion Care Services

Companion care services are well suited to individuals who are independent but require a helping hand to maintain their routines and enjoy their independence while living at home. Clients using companion care services range from new moms needing extra help, working parents with sick children, to seniors seeking companionship or household assistance.

Companion Care Services include (but are not limited to) the following:

  • Light housekeeping, laundry, shopping and meal preparation
  • Playing games, reading, watching television, and accompanying the client on trips
  • Providing relief for the family members who serve as the primary caregiver
  • Reminding the client about medications that must be taken
  • Accompanying the client on outings such as doctors’ appointments
  • Serving as a travel companion
  • Providing sitter services in a hospital, nursing home or other facility
  • Providing sitter services to parents wanting a day or evening out
  • Providing pet and houseplant care
  • Hospital to Home Transition Care

Hospital to home transition care services include (but are not limited to) the following:

  • Transportation from hospital to home and support to make the transition stress-free
  • Grocery shopping and meal preparation
  • Light housekeeping and laundry
  • Help with bathing, dressing, and personal care
  • Physical assistance and support to move safely around the home
  • Prescription pick-up and medication reminders
  • Transportation to and from follow-up appointments
  • Assistance Before and After Surgery

Before and After Surgery Services include (but are not limited to) the following:

  • Transportation to the hospital for procedure, surgery or treatment
  • Preparation of the home for client’s arrival
  • Transportation from the hospital to the home
  • Prescription pick-up and medication reminders
  • Grocery shopping and meal preparation
  • Light housekeeping and laundry
  • Personal care services including bathing, dressing, and grooming.
  • Alzheimer’s Disease and Dementia Care

Respite Care
